KVKK Application Form

This page explains how to submit your requests under Law No. 6698 and what information your application must contain. For the scope of these rights, see the KVKK Privacy Notice.

1. How to apply

You can submit your requests by any of the following methods permitted by legislation:

1. Written application. In writing to 30 N Gould St #59565, Sheridan, WY 82801, United States; or, in accordance with legislation, addressed to the data controller representative in Türkiye.
2. Registered e-mail address. An application to privacy@taxkolay.com from the e-mail address you previously notified to TaxKolay and which is registered in its systems.
3. KEP, secure electronic signature or mobile signature. An application created by a method permitted by legislation may be sent to the electronic application channel made available by TaxKolay.
4. Application module. Once a dedicated KVKK request module is live in the customer portal, that channel may also be used.

This list is not to be interpreted as narrowing the channels permitted by the Communiqué on the Procedures and Principles of Application to the Data Controller.

2. Information your application must contain

Your application must contain at least:

• name and surname;
• a signature, for written applications;
• Turkish ID number for citizens of the Republic of Türkiye; nationality, passport number or ID number, if any, for foreign nationals;
• residential or business address for service of notice;
• e-mail address and phone number for notification, if any;
• the subject of the request;
• information and documents supporting the request.

TaxKolay may request only such additional information as is necessary and proportionate to verify the applicant's identity.

3. Application form

• ( ) I want to learn whether my personal data is processed.
• ( ) I request information about my personal data that has been processed.
• ( ) I want to learn the purposes of processing and whether the data is used in line with those purposes.
• ( ) I request information about the third parties to whom my data is transferred, in Türkiye or abroad.
• ( ) I request correction of my incomplete or inaccurate data.
• ( ) I request erasure or destruction of my data where the conditions apply.
• ( ) I request that correction, erasure or destruction be notified to the recipients to whom the data was transferred.
• ( ) I object to an adverse outcome arising from analysis carried out solely by automated systems.
• ( ) I claim compensation for loss suffered due to unlawful processing.
• ( ) My other request under Article 11 of the KVKK is set out below.

4. Response time and security

TaxKolay concludes the application as soon as possible depending on its nature, and within 30 days at the latest. If the applicant's identity cannot be verified sufficiently, further identity confirmation may be requested in order to prevent the disclosure of another person's data.

The response may be sent in writing or electronically, according to the applicant's preference and security requirements.

5. Governing language

The Turkish version of this document prevails. The English translation is provided for information purposes only; in the event of any discrepancy between the two, the Turkish version applies.
